Intensive In-Home Services
(Provider for FAST FORWARD)

 

Many New Hampshire children, youth, and families who experience difficulties in day-to-day life due to serious emotional disturbances end up being moved out of their homes. They are often put into residential treatment facilities, psychiatric hospitals, juvenile justice facilities, or daytime programs that take them out of their schools and communities. Despite the best intentions and hard work of families and providers services are difficult to navigate and are fragmented into uncoordinated systems. New Hampshire has begun to make big strides to address these challenges with a new approach. The FAST Forward program – Families and Systems Together – is designed to serve youth with serious emotional disturbances and their families, whose needs aren’t met by traditional service streams and programs.  

ISO Program
(Individual Service Option)

 

The ISO is a program that offers structure and support in response to the fluctuating need of a youth. Services can be wrapped around a youth and his or her family while a youth remains in the family home. Services could include a combination of:

  • home-based counseling

  • transportation (to doctors' appointments, etc.)

  • some structured weekend time

Individualized Service Options (aka ISO) is a high intensity, multi-faceted program designed to serve families. The program tailors a variety of therapeutic, social and community-based services to the specific needs of a child and family, and strives to create a safe, stable, positive home environment for each child.


GOALS:

  • Ensure the safety of children

  • Achieve permanence within birth, adoptive, kinship, or foster families

  • Keep children connected to their families, communities, schools, and social networks

 

The program can:

  • Support families in their own homes

  • Reunify a child with his or her family

  • Place the child in a caring foster home

  • Provide older youth with the support, education, and skills they’ll need
    to transition to independence

 

ISO will collaborate with the family and referral source to develop a treatment plan that may include:

  • Individual/family counseling

  • Trauma-focused community based therapy/child-parent psychotherapy

  • Respite care

  • Crisis intervention/stabilization

  • Substance abuse assessments & treatment

  • Parenting education and in-home support

  • Foster home support

  • Short-term placement

HBT Program
(Home Based Therapy)

HBT services are designed as a similar format to ISO, however only require case management by a Bachelors level staff with clinical oversight. NFS case managers work closely with referral sources to determine ongoing support.

CHS Program
(Child Health Support)
​

MCHS is a service offered for families who require supervised or partially supervised visitation per DCYF guidelines. This includes transportation to the visits, and building rapport with the family served.

Lisa McCann, Program Director, received her Bachelor’s degree in Behavioral Science from Granite State College of NH and her Master’s degree in Social Work from the University of New Hampshire, Durham NH. Lisa is the Program Coordinator of the newly formed New Hampshire office, working with DCYF/JPPO offices to provide ISO services, intensive in-home services to provide families with structure and treatment to achieve their identified goals. She comes to Northeast Family services with over 15 years of experience working with children from birth to adulthood and families in the community based setting as well as experience in the residential setting for teens and adults. She has the experience and education to provide the support to families on behaviors and mental health and the effects these can have on the family system and identifying the strengths and needs of the families.
